I_________________________ understand that the rental period

for the above listed equipment is from _________ to _____________ and no longer.

If I wish to rent the equipment beyond the original the original period, I must obtain consent of Entertainment Specialists before the original rental period ends. If I am late in returning the rental equipment for any reason I agree to pay a late charge as follows: 1 day: 50% of the total rental fee. 2 days: 100% of the rental fee.

I understand that the total rental fee is not refundable under any circumstances. I understand that I am responsible to repair anything that gets broken, or replace anything that gets destroyed or stolen while the equipment is in my possession.

This rental agreement constitutes the entire agreement between myself and Entertainment Specialists and shall not be amended, altered or changed except by written agreement and signed by both parties. I hereby represent and warrant that I am of legal age, have received the above listed equipment, have received a copy of this rental agreement and have read and do fully understand all of the terms of this agreement.
